Transylvania Hospital’s Resources Navigation Program Earns National Recognition

BREVARD, N.C., March 17, 2010 – Recently selected as a finalist for a Case In Point Platinum Award, the Health Resources Navigation Program at Transylvania Regional Hospital is in good company. The Platinum Awards recognize excellent programs that demonstrate the importance of case management as an essential component of the healthcare system. Other nationwide finalists for these awards include Massachusetts General Hospital, Kaiser Permanente, Memorial Sloan-Kettering Cancer Center, CIGNA and GlaxoSmithKline.
 
Winners will be announced—and all finalists will be publicly recognized for their outstanding work—during an awards ceremony on April 20 at the National Press Club in Washington, D.C. Sponsored by Case In Point, the official magazine of the Case Management Society of America, the Platinum Awards program allows small, medium and large case management programs to share the important work they are doing to meet the needs of patients they serve.
 
Acting as a case manager, Health Resources Navigator Di Ucci oversees the TRH program and “navigates” uninsured, low-income patients identified by the Brevard hospital to help them find available resources in the community for the care they need. Funding for the TRH Navigation Program was provided by a pilot demonstration grant from the state of North Carolina, as well as a multi-year grant from The Duke Endowment.
 
Designation as a Platinum Award finalist is not the first recognition received by Transylvania Regional Hospital for its Health Resources Navigation Program. Last year, an article written about the program by Ucci earned a first-place award and was selected for publication in Case In Point. As part of the prize for winning first place, Ucci also received free registration and a scholarship to attend CMSA’s annual conference held in Phoenix, Ariz.
 
More importantly, because of its success, the TRH program is serving as a resource for other organizations that want to make a meaningful impact through similar programs. “We have received requests for information on the innovative services we provide from neighboring communities, as well as organizations located as far away as Portland, Ore.,” Ucci said.
